Entity name and type if you work through one
PRO affiliation and member ID where applicable
Copyright, ISRC, ISWC, ISBN, trademark or patent references you hold
Signed split or contributor agreements for collaborative work

Requirements vary by jurisdiction. Filings are prepared with, and reviewed by, your licensed professional of record — Metanated holds the record, not the license.
